We have produced a medical record card on which medication may be noted. This makes it easier to inform medical staff about medication when the patient may be unable to talk.
This is designed to be printed on an A4 page, which is then folded as indicated below. Printing a web page does not position the card correctly on the paper. It is better to transfer the .gif file to a drawing programme, and scale it to the 210mm width of a standard A4 page. When the card is exactly centred, it can be folded as required to the intended format.
Save the .GIF file in a suitable folder on your
computer. Web pages are saved as "Temporary Internet Files" in
the Windows folder. But it is easier to find a wanted file if it is saved
in the "My Documents" folder. Then use a suitable drawing
package to import the file and scale it to exact A4 width before printing.
(Atlantis allows exact scaling of pictures and may be useful here.)

These record cards are to help you know your normal measure of asthma. Do not let it become a chore. You can then use the record to monitor your control of asthma and as a warning if your asthma gets worse.
Peak Flow records help your Doctor to see if your medication is effective. It also shows how bad an asthma attack may be if you think you need to see him. Normal variation is about 5 %, that is about 25 if your normal reading is 500. A drop of 10 % probably means you should consult your Doctor. But some triggers can cause a brief drop of more than 10%, followed by a quick recovery ; it should not be necessary to consult your doctor about this, but make a note of it for your next regular visit.
This card is to help you record your PEAK FLOW readings for a month.
Most people have different readings first thing in the morning and during the evening.
Our card has space for three readings each day : first thing ; last thing ; and sometime during the day. The first reading should be as soon as you can after you wake up. The other two can be at any convenient time. One suggestion is after lunch.
The NOTES section is to record factors such as weather, changes of medication, special events such as holidays, trigger factors such as visiting a smoker or contact with animals. These are things which may affect the severity of your asthma. This record will help you decide what factors help and what factors are to be avoided.

This card has space for 24 readings each day. The first reading should be as soon as you can after you wake up. The others can be at any convenient time. You do NOT have to fill in a reading for every time slot. It does not matter if the peak flow readings are not exactly an hour apart. This is a guide for your information, to help you find things which may affect the severity of your asthma.
